Pathophysiology of residual sleepiness in patients with treated OSA

In treated obstructive sleep apnea (OSA), the pathophysiology of residual sleepiness remains poorly understood. Animal models suggest that it may involve neuronal damage due to intermittent hypoxia and sleep fragmentation. In a cohort of 122 OSA patients treated with continuous positive airway pressure (CPAP) referred for a wake maintenance trial, we present in this article the exploration of the determinants of alertness (objective) and sleepiness (subjective) assessed by the Epworth Sleepiness Scale. The results suggest that hypoxia and sleep fragmentation may be involved in different dimensions of residual hypersomnolence in treated OSA.
