We share a clinical case that demonstrates the value of an individualized therapeutic strategy for obstructive sleep apnea (OSA), focused on proper patient selection and a sequential and functional approach.
A 44-year-old patient with severe OSA (AHI: 44) and a clearly hypotonic phenotype, without obesity or multiple obstructions. Only a restrictive frenulum and a toneless tongue.
From the outset, we anticipated that she would be an ideal candidate for a combined approach:
- DISE-guided frenuloplasty, which unblocked tongue function.
- Personalized myofunctional therapy with Airway Gym, which increased tone and reduced the AHI to 28.
- Adapted to MAD, with excellent tolerance and results
Final result (May 2025):
AHI of 3.5, symptom-free, and with full adherence to treatment.
The most significant aspect was not only the clinical outcome; we anticipated it from day one.
It is a clear example of how personalized medicine, based on phenotype and function, can turn a limited indication into a healing opportunity.
